To make sure we're offering our students the best possible education and services, we want to learn more about their attitudes and behaviors regarding a variety of health issues through a voluntary MetroWest Adolescent Health Survey.
 

Protecting your teen’s privacy is important to us. The survey is anonymous, meaning your teen will not be asked to provide their name, and no one will know how they respond. However, if you still choose to opt your teen out of taking the survey, you must complete and sign the opt-out form below by Thursday, October 30th.
