Winter is right around the corner, and with it comes the flu season. As part of our continued effort to keep BVT a healthy environment during the emerging flu season, we are offering a flu vaccination clinic for our students on Friday, October 10th, during the school day. If you do not want to have your teen participate, do not fill out the form, and your teen will not receive the vaccine.

If you want your teen to participate, please indicate permission by completing the required Flu Insurance & Consent Information Form and e-mail the form to sbhc@valleytech.k12.ma.us by Friday, October 3rd. Forms can also be brought to the Main Office. The completion of this form is necessary for every vaccine recipient. If no insurance information is available, we ask parents/guardians to fill out as much of the form as possible, and your teen will still receive the vaccine. 

This voluntary program is available through our School Based Health Center in affiliation with Milford Regional Medical Center. Please note that your teen must also have a completed School Based Health Center Permission Form on file to receive the vaccine. If it is not on file, please complete and return this form as well, in order for your teen to participate in the clinic.
