We're committed to providing all our students with high-quality, confidential health care, and wellness services. We hope you find the following list of forms to be useful tools in managing many aspects of your teen's health during their time here at BVT. 

Before the start of school each year, all necessary forms should be submitted to the nurses' office. Our incoming freshmen will receive a student packet with all the necessary forms that can be returned together. All other students in grades 10, 11, & 12 need to submit the Nurse Emergency Form yearly. This updates emergency contacts as well as gives our school nurses permission to administer medication.
