Blackstone Valley Tech School Based Health Center (SBHC) provides onsite medical, nutrition, and mental health case management services. The Health Center is under the direction of a Medical Director, in conjunction with Milford Regional Medical Center. The Health Center offers convenient, teen friendly services in a confidential manner and is operational while school is in session.

 

 

 

 

Permission: If you are under 18 years old, one of your parents or guardians must sign the consent form before you can be seen at the School Based Health Center. However, if you do not have a signed consent form, the school nurse is always available to assess your medical problem.

Appointments: You can make an appointment by contacting the Office Coordinator at the School Based Health Center, located in the school’s health suite. When school is not in session (after hours or during vacations), we recommend you contact your primary care provider or local emergency room.

Payment: No student will be denied health services due to lack of insurance. If you have private insurance or Medicaid, the School Based Health Center will submit the necessary billing. If any health claim is denied, the SBHC will absorb the cost of your child’s visit.

Our goal is to treat and to prevent health and social problems that often interfere with academic attendance and success. Students can us the SBHC to receive preventative health care or treatment for acute/minor illnesses, psychosocial and emotional support. We are also available for health related questions, concerns, or support services if needed. The Health Center WILL NOT replace your primary care provider. Correspondence will be forwarded to your primary care provider.
